We reserve the right to change the terms of this Notice and make the new Notice provisions effective for all PHI we maintain, including ePHI stored in our EHR system. The revised Notice will be available upon request and posted in our facilities and on our website.

Complaints:

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
